The Care Coordinator (CC) assists with the care coordination of member care of both Utilization and Disease Management/Wellness. Under the direction and supervision of the Care Manager-RN, the Care Coordinator assists members by providing support, education and assistance in the prevention and/or maintenance of their disease and/or health and wellness state; by assisting in supporting member compliance with treatment plans; and by assisting in the collaborative coordination of care to reduce waste and inefficiency.

The role of the care management nurse is to coordinate continuity of care for patients often as a liaison between the patient’s family and healthcare organization, ensuring that the proper treatment is administered at the appropriate time to maximize health and well-being while also minimizing the need for hospitalization.

The Provider Relations Representative is responsible for communicating with all Providers and acting as the liaison between Careways and the Provider. Responsibility for growth of relationships with Physicians, Clinics, Networks and Hospitals. Provide training on the role and responsibility Careways plays within the primary care relationship.
