What we do. For members.

We provide our members with personalized care management and navigation assistance to help eliminate barriers to health and wellbeing. Our care coordination team members – care coordinators, registered nurses, pharmacists, and referral coordinators – work directly with members and their primary care providers (PCPs) to identify, understand, and take control of health risks and chronic diseases so that members have the best health outcomes possible.

As a part of the care management team, the care manager and care coordinator shares responsibility for the success of each member’s treatment plan. Any need for services or modifications to the treatment plan will be jointly agreed to by all members of the care management team. Care coordinators will use tracking tools, and recommend community resources, a web-based library, and educational programs to help members reach their health goals.
